A 28-year old non atopic woman was bitten on the right superior eyelid by a spider, later identified as a female Chiracanthium punctorium. Immediately the victim had a local severe stinging pain with blister and subsequent necrosis at the site of the bite, while a erythematous urticarial plaque extended in 8 hours over the whole head, neck and upper third of the chest. Furthermore, lethargy and hyperexcitability, with tearful crises and convulsions, were observed. Cutaneous and neurological conditions disappeared after 10 days of parenteral and oral corticosteroidal, antihistaminic and antibiotic therapy. The unusually aggressive behaviour of Chiracanthium punctorium, often involved in cases of human envenomation, is discussed.
