Lobular breast carcinoma within a fibroadenoma is a rare evenience usually incidentally found at histology after lumpectomy or biopsy for benign lump. Radiological patterns are not specific for diagnosis. Although high rate of multifocality and bilateral lesions in lobular carcinoma, the in situ tumors may be considered "risk factor" for the development of invasive breast cancer rather than a truly pre-neoplastic lesion. The surgical treatment might be the same of tumors not related to fibroadenoma, considering the breast-conserving-surgery as lumpectomy or wide biopsy followed by strict follow-up, the optimal treatment for this lesion. More aggressive approach might be evaluated just in selected cases with high familial risk or clinical evidence of bilateral or multifocal neoplastic foci.
