The clinical, histological and molecular findings observed in an Italian patient with malignant lymphoma and chronic pyothorax are described (pyothorax-associated lymphoma: PAL). Such a kind of neoplastic process characteristically develops a very long time (years or decades) after a therapeutic pneumothorax for tuberculosis. Latent EBV viral infection is demonstrated by the presence in the neoplastic cells of the EBV-related small RNAs EBER-1 and 2 along with the positivity for EBNA-2 and LMP-1. The present case is the first documented occurrence of PAL in Italy.
