We read with interest the consensus guidelines from the 15th St Gallen International Breast cancer conference. We would like to highlight some areas that contradict current best practise, so that clinicians can interpret these guidelines in the context of a consensus meeting rather than the evidence-based approach that is followed by most multidisciplinary guideline-preparing groups [1].
