The Authors report a case of a perineal lesion in a 9-year-old girl hit by a SUV while she was running out of her stepfather's shop. This kind of injury was compatible with a sexual abuse, supported by the history of her stepfather, a man with criminal records for this kind of crime. At first the inquiries strongly pointed to the sexual abuse and showed that the road accident had occurred unexpectedly, while the child was trying to elude her stepfather. However on demand of the medical examiner new witness evidence had been admitted together with a survey of the SUV. Only after these further inquiries the forensic pathologist, together with the gynecologist, excluded any hypothesis of violence and declared that the perineal injury was due to the running over. In such events a multidisciplinary approach is fundamental, led by the experience of the physicians involved.
