A 63 year-old male presented because of syncopal episode due to a large mass (7 × 5 × 7.5 cm) in the right atrium and ventricle, associated with pericardial effusion. Positron emission tomography/computed tomography (PET/CT) showed a hypermetabolic uptake (SUV max 22) in the heart but not in other anatomical sites. A cardiac biopsy documented a diffuse large B-cell lymphoma (DLBCL) expressing CD19, CD20, CD79a and CD79b with an ABC-Activated B Cell cell-of-origin (CD10-, BCL6+, MUM1/IRFA+) and c-MYC rearrangement. A diagnosis of primary cardiac lymphoma (PCL) was made for which R-COMP (rituximab, cyclophosphamide, vincristine, nonpegylated liposomal doxorubicin, prednisone) regimen was administered leading to a partial response (PR) that was followed by relapse soon after the completion of the sixth chemotherapy cycle.
