Ectopic pregnancy plays an important role in severe complications during the first trimester of pregnancy, causing 10% of all maternal deaths and 16% of hemorrhages in pregnancy. Abdominal pregnancy is a rare event occurring in 1.3% of ectopic gestations. We present the case of an abdominal pregnancy detected at the nineteenth week, presenting symptoms of hemoperitoneum and acute abdomen. According to our experience, an abdominal advanced pregnancy with no clinical symptoms is still a possible option. The pre-surgical, ultrasonographic diagnosis is somehow difficult and it is possible to find the abdominal fetus only at the time of laparotomy.
