Integrated home health care has been financed with substantial resources in the Nrrp with the aim of covering 10% of the over-65s. In 2024, the number of new patients increased by 141% compared to 2019, while the expenditure grew by 26%. Nrrp funding are still frozen within budgets allocated to regions, and the target has been achieved by increasing the number of low-complexity cases treated. Actually, the service does not ensure comprehensive and continuous home care over time for patients who require it, owing the lack of social and health care integration, while the Long-term Care reform suffers from insufficient funding. Considering the high variability of unit costs between Italian regions, there is an urge to adjust national tariffs based on patients’ risk stratification and linked to service standards so as to increase the levels of complexity care towards which the Regions should converge.
