In this paper we examine the gap between the North and the South of Italy in health and social services. We analyze differences in expenditure, quality and access to services after the decentralization of the ‘90’s with more local accountability and more central control. We argue that the gap has not been solved and some backward behaviours have spread all over the country. We conclude that the disparities of conditions of the South of Italy (the so-called “questione meridionale”) is a national matter, that has to be faced by national instruments and policies.
