Nasal cutaneous tumors rappresent 25% of the all head and neck cancers. Nasal pyramid has an aesthtetical and functional importance. The surgeon as to pay attention to restore the defect coverage, projection and three-dimension of the structure. We describe a case of caucasyan 82 years old man with plurire-current squamous cell carcinoma of the noise. The reconstruction was performed 8 months after excision by a forehead flap and 2 bilobed flaps for the alae reconstruction. We used an "L" shape bone graft to obtain more projection and nasal airway patency. We thing that the waiting time before reconstruction is foundamental to show eventual recurrences. The technique used permitted to achieve a continous clinical follow-up and an accettable morfo-functional result.
