Self-control and administrative-grotesque in psychiatric practice

Michel Foucault ha coniato l’espressione “grottesco amministrativo” per richiamare quei “discorsi di verità”, collocati al centro delle istituzioni mediche e giudiziarie, « che uccidono e che fanno ridere » . Attraverso l’espressione grottesco amministrativo, è possibile evidenziare quella zona di indistinzione, ove le pratiche discorsive della burocrazia e le azioni quotidiane della amministrazione pubblica saturano la vita corpora del potere dello stato. In questa zona sfumata, si collocano specifiche tecniche del corpo e una peculiare combinazione di potere si sovranità, potere disciplinare e potere di controllo. In questo articolo è proposta una descrizione etnografica del “grottesco amministrativo” nelle pratiche quotidiane di un Centro di salute mentale, nell’Italia centrale. Un Centro che è stato investito da una rapida trasformazione a causa di recenti cambiamenti nelle politiche di welfare. A questo scopo, sono richiamati i dialoghi fra pazienti e operatori relativi alla burocrazia; è inoltre descritto l’impatto delle nuove procedure amministrative del Sistema sanitario nazionale sull’attività del centro; e infine è evidenziato come tale impatto produca una combinazione perturbante di interazioni sociali focalizzate ripetitivamente sulle nuove procedure amministrative (nuova accountability, nuovo management, nuovi limiti di finanziamento, …) sulla esperienza di ansia, sulla gestione medico-psichiatrica del disturbo da attacco di panico, e su differenti tentativi di gestire una forma di vita in mutamento. TESTO INGLESE Michel Foucault coined the expression “administrative grotesque" to recall those "speeches of truth”, placed in the heart of our judicial and medical institutions, “that can kill and that make people laugh". Through the expression “administrative grotesque”, we could outline a blurred zone where the discursive practices of the bureaucracy, and the daily actions of the public administration, saturate the bodily life of state-power. In this blurred zone, we find specific techniques of the body and a peculiar mixture of power of sovereignty, disciplinary power and power of control. In this paper, I give an ethnographic description of the “administrative grotesque” in the daily practices of a Mental Health Centre, in Central Italy; a Centre that has been involved in a rapid transformation because of the recent change of welfare policies. To this aim, I recall the dialogues about the bureaucracy of the patients and the psychiatric workers. Then I describe the impact of the new administrative procedures of the NHS on the daily actions in the Centre. Finally I show how this impact produces an uncanny combination of social interactions focused respectively on the new administrative procedures (new accountability, new management, new budget limits, …), on the experience of anxiety and panic attack disorder, and on the various attempts to manage a changing daily life.
