Authors describe 7 cases of ischemic colonic disease observed from January 1991 to June 1993 at the Department of Surgery of the University of Perugia. All the patients were male, aged between 72 and 83 years. 3 Patients had undergone a previous abdominal aorta elective intervention and 1 Patient an emergency procedure because of abdominal aorta aneurism rupture; all the interventions had been performed by the Vascular Service of our Department. Clinical presentations of ischemic colitis were various, depending on the extension and severity of disease, but presence of abdominal pain and bloody diarrhea was demonstrated in the majority of our cases. According to our experience, colonoscopy is the preferable diagnostic procedure, in order to demonstrate the extent and severity of colonic lesions with bioptic confirmation. 4 Patients with ischemic colitis have been operated with emergency procedure, 3 Patients presenting with moderate disease were successfully managed with medical treatment. The intervention consisted in necrotic bowel removal in three cases and drain of mesocolic hematoma with lysis of adhesions in one case, in order to save the damaged but not necrotic segment of bowel
